Before the visit: choose your top question
Write the one thing you most want to understand. Add any other questions beneath it, then put them in your own order of importance. Your list can be a few words rather than a complete account of your pregnancy.
Gather what the office requested
Check the office’s appointment instructions for forms, records or other items they want you to bring. Keep your own notes and a current medicine list handy if relevant. Ask the office about any preparation you are unsure of.
Write preferences as conversation starters
You may want to discuss how your pregnancy or birth preferences affect upcoming decisions. Try prompts such as “What options should I understand?” and “What would change this plan?” Your preferences can change as you learn more.
During the visit: leave space for answers
Keep your question list open and take brief notes in language that makes sense to you. Before leaving, clarify any next steps, who will arrange them, and how to reach the office with another question.
After the visit: organize the next step
Put dates the office gives you in your calendar, keep the visit notes together, and add unanswered questions to your next list. Follow the care plan you discuss with your clinician, rather than an app-generated schedule.
Use the free visit worksheet
The worksheet has spaces for your questions, observations, conversation notes and agreed next steps. You can print a blank PDF without signing up or buying the app. If you type into the browser worksheet, print or save it before closing; the page does not save your notes.
